Use this form to add an additional driver to your existing Brentacre Insurance policy.

It is essential that you provide all material information likely to influence the acceptance and assessment of this insurance. If you have any doubts as to whether a fact is material, it should be disclosed. Failure to disclose any material facts may invalidate your policy or result in your policy not operating fully.

Important: What You Will Need For Each Driver

Before adding a driver below, please make sure you have the following ready:

  • Their DVLA licence information. You can either upload a PDF copy of their licence summary, or enter a licence check code. The check code is 8 characters, case sensitive and valid for 21 days from when it is created.
  • A copy of the front of their photocard driving licence. A clear photo or scan is fine.

It is an offence under the Road Traffic Acts to make any false statements for the purpose of obtaining a certificate of motor insurance.

How to obtain your licence summary or check code

For each driver, you will need either a PDF of their DVLA licence summary or a licence check code:

  1. Visit www.gov.uk/view-driving-licence
  2. Enter the driver's licence number, National Insurance number and postcode
  3. Click "Share your Licence Information" to create a check code
  4. You can either enter this check code in the relevant driver section above, or save the summary page as a PDF and upload it instead

Please note the check code is case sensitive and only valid for 21 days, so make sure you submit this form soon after creating it.

You will also need to upload a photo or scan of the front of each driver's photocard driving licence. A clear phone photo is fine, as long as all details are readable.
